We participate with most major insurances. If we participate with your insurance you are expected to pay your co-pay and any applicable out-of-pocket expenses at time of service. We use the information provided by your insurance company on the date of your visit to calculate your share.
If your injury for which you are being seen occurred on the job, we will need prior authorization from authorized company personnel to bill your visit to workers’ compensation. If we do not have the authorization you will be responsible for all charges related to your visit.
While we try to handle prescription refill requests as soon as possible, please allow up to 48 hours for refills to be called in. Please notify us before you run completely out of your medication. Some medications may require that you be rechecked by a provider before the medication can be refilled. You will be notified if a recheck is necessary.
If we do not participate with your insurance or you have no insurance you are expected to pay in full at time of service.
